Story Mode
Being the default mode, and prior to version 1.1, the only mode, this mode does not have an official name. It is accessed by clicking the door in the bathroom without first consuming the pills on the counter. This mode consists of three stages.
Round 1
Round 1 consists of a simple game of chance, with no items except the Shotgun. The Player and the Dealer begin with two charges each. The number and type of shells is predetermined, but their order is random. The first round has three shells: one live, two blank. The second round has five shells: three live, two blank. If the player loses this stage, they are revived in the Bathroom by the Doctor. If the Player wins this round, the game proceeds to round 2.
Round 2
Round 2 introduces items into the game. Before every round, both sides draw two random items from the item box each. While drawing before the second round, a bloodied waiver signed by GOD comes out, is seen by the Player for a brief moment before being returned to the box. In this round, the number and type of shells per round are random, except for round one. Round one always has two shells: one live, one blank. The Player and Dealer have four charges each. Like round 1, if the player loses this round, they are revived in the Bathroom by the Doctor. If the Player wins this round, the game proceeds to round 3.
Round 3
This is the final round. On the game machine, it is depicted as a skull and crossbones, as losing this stage means death for the Player. In this round, the number and type of shells per round are entirely random. The number of items drawn per round is increased to four. Both the Player and Dealer have four normal charges and two faded charges. If only the faded charge(s) remain, the cables above that participant’s side of the game machine are cut, signifying the point of no return where the defibrillator will no longer work, and so any further damage is fatal. If the Player wins this round, the results of the game will be shown after the Player receives a briefcase full of money, the amount depending on the items they had used.
Double or Nothing mode
Officially known as Double or Nothing mode, also colloquially called endless or pills mode, this mode was added in version 1.1. It is aimed at those who have played story mode previously, with tutorial sequences absent. It is accessed by first consuming the pills in the Bathroom before leaving.
In the Hallway, the Doctor is notably absent, implying that there is no one to revive the player should they run out of charges. if the Player dies here, instead of the Doctor reviving them, the screen will become black before returning back the Bathroom to start a new run from scratch. This mode also has three stages, however successfully completing a set of stages instead shows the player’s currently accrued winnings, presenting the option to play another set for double this amount. Should the player accept, another set begins. Should the player decline, the results are shown.

The Hallway which the Doctor is notably absent from. In each stage, the following are random:
- Number and type of shells: 2-8
- Items per round: 1-5
- Charges: 2-4
Both the player and dealer get matching charge counts. The items counts are matched, but the drawn from the box are random per participant.
